The form is only for registration of IP addresses of a site for free on-line access to subsctibers to CSJ journals. This should only be completed by the librarian or duly authorised member of staff at the organization.

A site is defined as one institution or company in one city, or within a five mile (eight km) radius of the subscriber's address. IP addresses that cover only this definition should be given.

• Please do not use this form if you wish to notify us of any change. Instead, send the changes to ejournals@chemistry.or.jp quoting your customer number and full address of your organization.
